If are a registered block club that is interested in having an alley near your home cleaned, please fill out this form.

Please complete this form in its entirety to submit a request for an alley clean up. In order to be considered, you must belong to an active Block Club or Neighborhood Association. If your organization is requesting multiple alley clean ups, a new request form must be used for each alley.
